ABSTRACT

Abstract Introduction Primary ciliary dyskinesia (PCD) is a rare inherited disease associated with impairment of mucociliary transport and, consequently, with a high incidence of chronic rhinosinusitis. For patients with chronic rhinosinusitis who remain symptomatic despite medical treatment, endoscopic sinus surgery is a safe and effective therapeutic option. However, to date, no studies have been found evaluating the effect of surgery on the quality of life associated with the effect on olfaction and nasal endoscopy findings of patients with primary ciliary dyskinesia and chronic rhinosinusitis. Objective To describe the effect of endoscopic sinus surgery on the quality of life, on olfaction, and on nasal endoscopy findings of adults with PCD and chronic rhinosinusitis. Methods Four patients who underwent endoscopic sinus surgery were included. The Sinonasal Outcome Test-22 (SNOT-22) score, the Nasal Obstruction Symptom Evaluation (NOSE) questionnaire, and the Lund-Kennedy score were collected preoperatively and at 3 and 6 months postoperatively. The olfaction as assessed with the University of Pennsylvania Smell Identification Test (UPSIT), which was administered preoperatively and 3 months postoperatively. Results A total of 4 patients with a mean age of 39.3 years old (3 men and 1 woman) completed the study. All patients showed clinically significant improvement in the SNOT-22, NOSE, and Lund-Kennedy scores at 3 months postoperatively, and this improvement was sustained throughout the follow-up period. However, olfaction did not improve after surgery. Conclusion The endoscopic sinus surgery treatment of chronic rhinosinusitis in adults with PCD was associated with improvement in quality of life and endoscopic findings. However, no improvement in olfaction was demonstrated. Studies with a larger number of patients and control groups should help confirm these findings.

INTRODUÇÃO: A rinossinusite crônica (RSC) é uma doença comum na população, com documentada repercussão na qualidade de vida e com alto custo direto em saúde pública que engloba consultas médicas, exames complementares e radiológicos, internações hospitalares, cirurgias e tratamento medicamentoso. Apresenta também custos indiretos como diminuição da produtividade no trabalho e absenteísmo. Os dados epidemiológicos sobre rinossinusite crônica são escassos; e as definições de rinossinusite crônica estabelecidas, as metodologias das pesquisas e as taxas de respostas diferem muito entre si. A pesquisa epidemiológica da RSC é importante para avaliar sua distribuição, analisar seus fatores de risco e fornecer dados para promoção de políticas de saúde pública, entretanto não existem dados epidemiológicos sobre a prevalência desta doença em nossa população. MÉTODO: Foi realizado um inquérito transversal de base populacional (survey) com plano de amostragem complexo realizado em dois estágios: setor censitário e domicílio. Entrevistas foram conduzidas pessoalmente, através de entrevistadores treinados, em 2003 indivíduos com idade de 12 anos ou mais, residentes da cidade de São Paulo. O questionário incluiu o diagnóstico de rinossinusite crônica segundo os critérios epidemiológicos estabelecido pelo EP3OS. Dados demográficos, diagnóstico médico autorreferido de doenças respiratórias (asma, sinusite, rinite), tabagismo, renda familiar, nível educacional e características do domicílio também foram incluídas. RESULTADOS: A taxa de resposta total foi de 87,8%. A idade média foi de 39,8 anos (DP= 21, 12-92), 45,33% do sexo masculino. A prevalência da rinossinusite crônica na cidade de São Paulo foi de 5,51% (IC 95%=3,99-7,58). Não existiu uma diferença estatisticamente significativa na prevalência segundo o sexo. Foi encontrada uma associação estatisticamente significativa do diagnóstico de RSC com o diagnóstico de asma (OR=3.88), de rinite (OR=5,02)...


INTRODUCTION: Chronic rhinosinusitis (CRS) is a common disease with proven repercussions on quality of life and a high burden of direct costs to public health, which included physician visits, laboratory tests and medical imaging, hospital admissions, surgical intervention, and medical treatment. It also carries indirect costs, such as decreased productivity in the workplace and absenteeism. Epidemiological data on CRS are scarce, and the established definitions of CRS, study methods, and response rates vary widely. Epidemiological research into CRS is important to assess its distribution, analyze its risk factors, and provide data to subsidize public health policies; however, there are no epidemiological data on the prevalence of this disease in our population. METHOD: This was a cross-sectional population-based survey with a complex cluster sampling plan carried out in two stages: census sector and household. Interviews of 2,003 individuals, aged 12 years or older and living in the city of São Paulo, were conducted face-to-face by trained investigators. The questionnaire included diagnosis of CRS according to the epidemiological criteria established by EP3OS. Demographic data, a self-reported history of physician-diagnosed respiratory diseases (asthma, sinusitis, rhinitis), smoking, family income, educational attainment, and household characteristics were also included. RESULTS: The overall response rate was 87.8%. The mean age was 39.8 years (SD = 21 years; range = 12-92), and 45.33% were male. The prevalence of CRS in the city of São Paulo was 5.51% (95%CI = 3.99-7.58). There was no statistically significant difference in prevalence between the sexes. Statistically significant associations were found between diagnosis of CRS and diagnosis of asthma (OR=3.88), of rhinitis (OR=5.02), and belonging to the low-income subgroup (OR=2.28) The prevalences of CRS according to smoking status (p=0.43), tobacco intake in pack years (p=0.26)...

Obstrução nasal é uma queixa frequente. Quando causada por um desvio de septo nasal, a septoplastia é cirurgia de escolha para tratamento destes pacientes. O questionário NOSE é um instrumento para avaliar a qualidade de vida doença-específica dos pacientes após intervenções para tratamento da obstrução nasal. OBJETIVO: Avaliar o impacto da septoplastia na qualidade de vida doença-específica dos pacientes com obstrução nasal secundária a desvio de septo. Desenho: Prospectivo. PACIENTES E MÉTODOS: Pacientes submetidos à septoplastia com/sem turbinectomia após ausência de melhora clínica ao tratamento medicamentoso foram avaliados através do NOSE antes e 3 meses após cirurgia. Avaliou-se a melhora na pontuação total, a magnitude do efeito da cirurgia na qualidade de vida doença-específica e a correlação entre a pontuação pré-operatória e a melhora pós-operatória. RESULTADOS: Quarenta e seis pacientes foram incluídos no estudo. Observou-se melhora estatisticamente significativa entre a pontuação do questionário NOSE pré-opera¬tória (md = 75, IIQ = 26) e após três meses (md = 10,IIQ = 20, p < 0,001.T-Wilcoxon). A cirurgia resultou numa magnitude de efeito padronizada de 3,07. Mostrou-se uma forte correlação entre a pontuação pré-operatória no questionário NOSE e a melhora da pontuação no pós-operatório (r = -0,789, p < 0,001, Spearman). Não se encontrou diferença entre a melhora na pontuação segundo sexo. (p = 0,668, U-Mann-Whitney). CONCLUSÃO: A septoplastia resultou numa melhora da QV doença-específica estatisticamente significativa.


Nasal obstruction is a common complaint in the population. When caused by a deviated nasal septum, septoplasty is the procedure of choice for treating these patients. NOSE is a tool for assessing the disease-specific quality of life related to nasal obstruction. AIM: To assess the impact of septoplasty on patients with nasal obstruction secondary to deviated nasal septum based on the disease-specific quality-of-life questionnaire. Design: Prospective. METHODS: Patients undergoing septoplasty with/without turbinectomy after no clinical improvement with medical treatment were assessed by the NOSE questionnaire before and 3 months after surgery. We evaluated the surgical improvement based on total score, the magnitude of the surgery in the disease-specific quality of life and the correlation between the preoperative score and postoperatively improvement. RESULTS: Fourty-six patients were included in the study. There was a statistically significant improvement in the preoperative NOSE score (md = 75, IQR = 26) and after three months (md = 10, IQR = 20) (p < 0.001.T-Wilcoxon). The standardized response mean was 3.07. We found a strong correlation between the preoperative score in the NOSE questionnaire and improvements in the postoperative period (r = -0.789, p < 0.001, Spearman). No difference was found in improvement scores by gender. (p = 0.668, U-Mann-Whitney). CONCLUSION: Septoplasty resulted in a statistically significant improvement in the disease-specific QOL questionnaire.

A rinossinusite crônica é uma doença de etiologia não definida que impacta significativamente a qualidade de vida dos pacientes. A sinusectomia endoscópica foi demonstrada como um tratamento eficaz em melhorar a qualidade de vida dos pacientes em outros países; contudo, não existem estudos nacionais. OBJETIVO: Avaliar a associação cirurgia endoscópica nasossinusal com a qualidade de vida doença-específica dos pacientes com rinossinusite crônica com e sem polipose nasossinusal pelo SNOT-20. Desenho: Estudo prospectivo. PACIENTES E MÉTODOS: Pacientes submetidos à sinusectomia endoscópica após ausência de melhora ao tratamento medicamentoso foram avaliados pelo questionário SNOT-20p antes e 12 meses após a cirurgia. Avaliou-se a melhora na pontuação total e nos cinco itens considerados mais importantes por cada paciente. Avaliamos também a presença de correlação entre a pontuação pré-operatória e a melhora pós-operatória e se havia diferença entre a melhora segundo sexo. RESULTADOS: Incluímos 43 pacientes com idade de 44 (19), md (IIQ); e 60,5% (26/43) do sexo masculino. Os pacientes apresentaram melhora estatisticamente significativa no SNOT-20 e SNOT-20 (5+), e correlação entre a pontuação pré-operatória e a melhora da pontuação (p<0.001). Não houve diferença entre melhora da pontuação na qualidade de vida segundo o sexo. CONCLUSÃO: A sinusectomia endoscópica em pacientes com rinossinusite crônica apresenta associação com melhora da QV doença-específica estatisticamente significativa.


Chronic rhinosinusitis is a disease of undefined etiology that significantly impacts the quality of life of its patients. Various studies carried out in countries other than Brazil have shown endoscopic sinus surgery as an effective means of treating this condition. OBJECTIVE: This study aims to analyze, with the aid of SNOT-20, the association between endoscopic sinus surgery and disease-specific quality of life of Brazilian patients treated for chronic rhinosinusitis accompanied or not by nasal polyps. MATERIALS AND METHODS: This prospective study enrolled patients submitted to endoscopic sinus surgery after drug therapy failed to improve their symptoms. They were assessed based on questionnaire SNOT-20p before and 12 months after surgery. Improvement on total scores and on the five items deemed more important by each patient were assessed. The study also looked into the correlation between preoperative scores and postoperative improvement and if there were any gender-related improvement differences. RESULTS: Forty-three patients aged 44 (19), md (IQR), 65% of whom (26/43) were males. Statistically significant improvement was seen on SNOT-20 and SNOT-20(5+) and a correlation was established between preoperative scores and postoperative improved scores (p<0.001). No gender-related differences were observed in quality of life. CONCLUSION: Endoscopic sinus surgery in patients with chronic rhinosinusitis is associated with statistically significant improvements in disease-specific quality of life.

A tonsilectomia, associada ou não à adenoidectomia, continua a ser um dos procedimentos cirúrgicos mais realizados mundialmente, incidindo principalmente sobre a população pediátrica. OBJETIVO: Investigar o impacto do uso da amoxacilina por 7 dias na recuperação pós adenoamigdalectomia, comparando os resultados com um grupo controle. Tipo de Estudo: Estudo prospectivo randomizado controlado com 120 pacientes. PACIENTES E MÉTODO: Os pacientes foram randomizados ao tempo da cirurgia para receber um curso de 7 dias de amoxacilina associada a analgésicos ou apenas analgésicos. Durante a primeira semana de pós-operatório foram avaliados o grau de dor, aceitação da via oral, náuseas e vômitos, febre e retorno às atividades. RESULTADOS: Somente no 4º pós-operatório o grupo recebendo antibiótico teve uma diferença estatística significante no grau de dor. Não houve diferença entre os dois grupos para outros dados analisados. CONCLUSÃO: Considerando os resultados do nosso estudo e revisando a literatura sobre o uso de antibióticos, nós concordamos que não há nenhuma melhora na recuperação dos pacientes submetidos à adenoamigdalectomia após o uso de amoxicilina por 7 dias.


Tonsillectomy with or without adenoidectomy still is one of the most commonly performed surgical procedures in the world, mostly in the pediatric population. AIM: to study the impact of amoxicillin for 7 days in post-adenotonsillectomy recovery, comparing results with a control group. Study type: prospective, randomized, controlled study with 120 patients. PATIENTS AND METHODS: the patients were randomized according to surgery time to receive 7 days of amoxicillin associated with pain killers, or analgesic alone. During the first week of postoperative, we assessed the level of pain, oral intake acceptance, nausea and vomits, fever and return to daily activities. RESULTS: It was only in the fourth post-operative day that the group receiving antibiotic agents showed a statistically significant difference as far as pain is concerned. There was no difference between the two groups for other data analyzed. CONCLUSION: considering the results from our study and reviewing the literature on the use of antibiotic agents, we agree that there is no improvement in patient recovery after adenotonsillectomy with the use of amoxicillin for 7 days in the postoperative.
